INTENSIVE 4 - ADVANCED SPINAL DIAGNOSIS (CERVICAL)

The cervical spine often attracts greater concern and scrutiny than other spinal regions, particularly when the possibility of neck manipulation is raised. This Intensive will help you to understand the challenges of accurate diagnosis and decision-making in this complex area.

As you will learn, most patients with cervical disc pain present with widespread symptoms that do not look like radiculopathy. Many disc lesions simply manifest as scapula, axilla or chest pain. Some express disc pain through repeated bouts of torticollis over many years. Would you recognise this? Even in cases with nerve root involvement the patient often complains of non-dermatomal symptoms, which are frequently misdiagnosed as a shoulder complaint (such as a 'supraspinatus tendonitis'). It is your job to make the diagnosis and triage accordingly.

Whiplash patients often face an uncertain future of pain and neurological disability. But what are the mechanisms of this? Why do some individuals experience dizziness, cognitive dysfunction, visual disturbances and tinnitus? What role might the facet joints, multifidus muscles and vestibular system play in such a diverse set of symptoms?

In the Advanced Spinal Diagnosis Intensive you'll learn how to use the orthopaedic and neurological examination as more than just a search for end-stage pathology. That’s why this Intensive consistently reinforces the neurological mechanisms that underpin the control of posture and joint stability. You will also learn the essentials of interpreting MRI scans - one of the most valuable tools for making your diagnosis, and your prognosis.

SOPHISTICATED DIAGNOSIS
BECOME A TRUE EXPERT IN SPINAL DISORDERS
This is a stand-alone seminar that is part of the Advanced Spinal Diagnosis series. It is not necessary to have attended any other Intensive beforehand.

The program is presented in a 'case-based' format. You will hone your diagnostic thinking by solving 4 challenging cases and learning along the way. Each clinical scenario represents a common situation that you'd be expected to have great proficiency in managing.

The topics include:
  1. CERVICAL DISC LESIONS WITHOUT NEURAL COMPRESSION
How does a cervical disc lesion present if there are no arm symptoms to suggest nerve involvement? What is the relationship between disc pain and torticollis? How can you tell if a wry neck originates in the facet joints or from a disc lesion? How does disc degeneration influence the balance system? What is relevant on MR imaging?
  1. CERVICAL DISC LESIONS WITH NEURAL COMPRESSION
What are the most important MRI features when assessing patients with cervical radiculopathy? Which examination features are reliable prognostic indicators, and which are simply a distraction? How long should you persist with conservative care before considering surgery? What does your treatment actually achieve?
  1. CERVICAL FACET JOINT LESIONS AND WHIPLASH
Why do facet joints become chronically painful? What tissue lesions occur in Whiplash-Associated Disorders? What is the role of muscle fatty infiltration? How do central pain mechanisms take hold? What is the role of stress and anxiety? How do adjustments create greater spinal stability and control?
  1. CERVICAL CORD LESIONS
How does a cervical myelopathy present? What are the tell-tale signs of spinal cord involvement? What is the role of MRI, and what would you look for? How would you know when surgery becomes necessary?
In each of these scenarios you’ll be guided through a clinical thinking process and introduced to the most important diagnostic distinctions. You will learn how to interpret the key MRI findings and build your confidence in ordering advanced imaging. You will understand how and when to make the decision to refer, and appreciate the pros and cons of surgery.

The currency of respect in health care is diagnostic expertise. But you also need a strategy to demonstrate your value as a problem solver (particularly to the patient's GP). This Intensive will equip you with the knowledge and tools to skilfully navigate the complex world of spinal diagnosis. You’ll also take away useful skills for communicating your knowledge and solidifying your positioning as the spine expert.
CPD POINTS AWARDED
This program has been assessed by the Australian Chiropractors Association, a body recognised by the Chiropractic Board of Australia, and allocated 7.5 CPD hours.

It has also been recognised by the International Academy of Neuromusculoskeletal Medicine (IANM) for 7.5 hours of board certification-level education.
