INTENSIVE 5 - ADVANCED SPINAL DIAGNOSIS (THORACIC)

When studying the spine, the thoracic region receives considerably less attention than its cervical and lumbar neighbours. Yet it is one of the most important areas for you to master.

The thoracic spine is a frequent source of mechanical pain, whilst also a potential site of covert pathology. During this Intensive we shall look at the many ways that the thoracic region might conspire to confuse us with back pain, chest pain and arm pain presentations. How does a thoracic disc lesion behave? When should we suspect spinal cord involvement? How would I differentiate between musculoskeletal causes of chest pain and something more sinister? How does a compression fracture present and what can we do about it?

However, this Intensive is not just about pathology. We shall look at the contemporary theories underpinning scoliosis development, particularly the role of the brain and the balance system. We will also tackle thoracic outlet syndrome - a condition that is still poorly understood by many practitioners. In particular, we'll learn how to differentiate this disorder from other neurological conditions that might present similarly (such as cervical radiculopathy).

You will come away from this seminar with a new perspective on this often misunderstood and poorly appreciated area of the spine. So join Dr Anthony Nicholson for an experience that will greatly increase your confidence and your diagnostic expertise.

SOPHISTICATED DIAGNOSIS
BECOME A TRUE EXPERT IN SPINAL DISORDERS
This is a stand-alone seminar that is part of the Advanced Spinal Diagnosis series. It is not necessary to have attended any other Intensive beforehand.

This event is presented in a 'case-based' format. You will hone your diagnostic thinking by solving 5 challenging cases and learning along the way.

The topics include:
  1. THORACIC DISC LESIONS
How does a thoracic disc lesion present? Do you recognise the warning signs of spinal cord involvement? What is the role of MRI, and what would you look for? How would you know if surgery is necessary?
  1. MUSCULOSKELETAL CHEST PAIN
How do you exclude cardiac, pulmonary and other pathological causes of chest pain? How do the costotransverse and costovertebral joints cause chest wall pain? What is 'cervical angina' and could the pain be due to cervical nerve root irritation?
  1. SCOLIOSIS
Is scoliosis a 'mechanical' disorder or a 'neurological' one? What does the contemporary neuroscience literature tell us about the role of the brain in this condition? Is it actually a form of dystonia? Or perhaps a disorder of the vestibular system? Would you recognise the tell-tale warning signs of an underlying pathology, such as syringomyelia, Chiari malformation, tethered cord or intramedullary tumour? How would you assess a young patient for signs of impending progression? What can your treatment actually do about scoliosis?
  1. OSTEOPOROTIC COMPRESSION FRACTURE
How does an osteoporotic compression fracture typically present? Why does a spinal compression sometimes produce pain in the iliac crests or lateral rib cage and not in the spine itself? What is the significance of the DEXA and how do you interpret it? What is the role of exercise and calcium supplementation in osteoporosis management?
  1. THORACIC OUTLET SYNDROME
What is the difference between 'vascular' and 'neurological' thoracic outlet syndrome (TOS)? How do you differentiate between TOS and spinal radicular causes? Which clinical tests are truly valid? What does a conservative management approach look like?
In each of these scenarios you’ll be guided through a clinical thinking process and introduced to the most important diagnostic distinctions. You will learn how to interpret the key MRI findings and build your confidence in ordering advanced imaging. You will understand how and when to make the decision to refer, and appreciate the pros and cons of surgery.

The currency of respect in health care is diagnostic expertise. But you also need a strategy to demonstrate your value as a problem solver (particularly to the patient's GP). This Intensive will equip you with the knowledge and tools to skilfully navigate the complex world of spinal diagnosis. You’ll also take away useful skills for communicating your knowledge and solidifying your positioning as the spine expert.
CPD POINTS AWARDED
This program has been assessed by the Australian Chiropractors Association, a body recognised by the Chiropractic Board of Australia, and allocated 7.5 CPD hours.

It has also been recognised by the International Academy of Neuromusculoskeletal Medicine (IANM) for 7.5 hours of board certification-level education.
